I just got the survey today. I pretty understand it except the options questions. Do I just put down the level I purchased or what do I put down for options?

I think the options refers to how you want to "spend" the amount over your basic pledge level.
For example:
Enerii Pledged $132.
This makes him eligible for the book and listed swag at the $100 level and he included $32 for options.
He selects the Jump Drive option for $10 (instead of the included CD-ROM he gets the thumb drive version of the rules)
He also selects one of the T-Shirts for $22.
Total = $132
