I'd like to propose a minimal 5.09 patch.
As far as the mechanical range of the difficulty goes I think the 5.09 system could be patched up a fair bit by simply resetting the difficulty alterations* so that they never decrease the difficulty. 1d at close range is plenty of, "can't miss at this range," all by itself. So aimed fire would be 0d and snap fire would be +2d.
Of course if you and your target are both under full cover you're still looking at a -9. I'm picturing two guys pointing their guns over a low wall while covering their eyes now.
I still believe that applying a +7 to the melee number mostly fixes the existing system but it might be more consistent to make defense work like cover or Rolemaster and GURPS. Just allow the defender to lower their own skill and the attacker's by the same amount.
