Is there a character record sheet in the book?

I seem to recall a bunch of sheets we can copy and fill out, but I can't find one for characters. Is there a sample sheet in the book for characters?

Inside the front cover is a blank character card, (front and back)

Also pages 106 and 107 has the character cards for humans and nonhumans.

Human character sheet is p. 3 there, and non-human is p. 4.

Hint: they only included planets to size A (10), then size 15 and size 20. However the Master Text Old included the maps for even larger planets between sizes 10 and 15.
