T5 "Passport" Character Sheet example - Nathler

Here's a character sheet from an adventure I ran in November 2012. The format is simple, and is folded to "passport" size.

The outer cover is simply an Imperial logo.

The interior is the UPP and the skills list, with whitespace for the player to note any equipment, cast, and anything. Additional information and assets (a TAS membership card, for instance) can be clipped to the interior.

Finally, on the back is the character's name and background -- things that someone else can know about this character through acquaintance.
